Join our team as the Intensive Care Coordinator with Wraparound (ICCW) Coordinator engages families in the Wraparound process, helping them to formulate a strength- based Family Plan, as well as an Individual Plan of Service, detailing their natural supports, needed resources and goals, plans to accomplish them; and assist them in assembling a Child and Family Team to support this plan. The ICCW Facilitator shall be responsible for administering the Wraparound planning process as trained by the Michigan Department of Health and Human Services (MDHHS) model. The ICCW Coordinator incorporates LifeWays mission, vision, and values into all decision-making processes.
This role is ideal for someone who is compassionate, organized, culturally competent, and skilled at bringing people together.
What Youll Do

Empower Families: Facilitate the Wraparound process to develop strength-based Family and Individual Plans of Service.

Coordinate Care: Connect families with natural supports, community resources, and healthcare providers to ensure continuity of care.

Lead with Compassion: Engage with children and families in homes, schools, and community settings, helping them reach their goals.

Advocate & Collaborate: Work with Child & Family Teams, community partners, and other providers to support family needs.

Crisis Support: Assist with intervention, discharge planning, and smooth transitions (e.g., inpatient to community care, school to work).

Document & Track Progress: Maintain accurate, timely records in our electronic health record system.
