Job Summary: Multidisciplinary oncology care is a comprehensive approach to a patients care that involves more than one specialty physician. In a multidisciplinary setting, physicians of different specialties come together to diagnose and treat patients. Having multiple specialty physicians in one place at one time allows them to collaborate in real-time and develop comprehensive, personalized patient plans. The Navigator role is to provide coordination and support to patients and physicians in the Multidisciplinary clinic.

Job Duties / Responsibilities:
Schedule and coordinate a patients care from the time of referral, and throughout the diagnosis and early treatment period.Review incoming referrals and appropriately schedule work-up for New Patient visit.New Patients must be contacted within 3 business days of receipt of referral and scheduled within 7 business days of being received.Prepare patients chart for office visit obtain medical records, order diagnostics, compile patient information, and provide information to the patients doctor prior to patients clinic visit.During clinic be able to present information to provider and to act as Navigator for patient.May assist physician with in-office procedures.May assist with rooming patients, obtaining vitals, room patients and enter required EMR documentation.Promptly carrying out urgent referrals/orders. Urgent referrals should be sent on the same or next business day.Carry out patient plans following the multidisciplinary conference.Follow up on all outgoing orders to ensure external clinic receives orders. Ensure patients are scheduled for diagnostic testing in a timely manner.May upload medical records on assigned patients.Provide education and support to patients.Return patient calls/Canopy tickets by 3PM on day of incoming call.Keep provider informed of patient progress, issues, or concerns.Attend the weekly Multidisciplinary conference for your assigned specialty. Enter assigned patient data on spreadsheet each month.Maintain good working relationships with internal and external multidisciplinary staffMust work well independently and as a team memberMaintains good communication with patients, referring providers, Chest Clinic doctors, co-workers and supervisors. Other tasks as assigned by supervisor.
