Job Summary:
The Bilingual Care Manager serves as a key member of the clinical care team at MindCure, Inc., providing culturally competent, trauma-informed support to clients navigating mental health services. This role includes client engagement, care coordination, appointment follow-up, and support in accessing community-based resources. The Care Manager ensures that both English and Spanish-speaking clients receive equitable and accessible care, contributing to improved mental health outcomes.

Essential Functions and Responsibilities:

Conduct intake screenings, needs assessments, and service coordination for new and existing clients in both English and Spanish.

Serve as a point of contact for clients and families, helping to schedule appointments, provide reminders, and address barriers to care.

Collaborate closely with therapists, psychiatrists, case managers, and other providers to ensure continuity and consistency of care.

Assist clients with accessing social services, including housing, food security, transportation, financial aid, and other community resources.

Translate or interpret for therapy sessions, documents, and clinical communications when appropriate and in alignment with HIPAA regulations.

Track client progress, service utilization, and follow-up needs using the EHR (e.g., TherapyNotes).

Support crisis management and safety planning under the supervision of licensed clinicians.

Advocate for culturally and linguistically responsive services in care planning.

Participate in team meetings, case conferences, and quality assurance activities.

Maintain documentation in compliance with agency, county, and state requirements.

Required Qualifications:

Associate or Bachelors degree in psychology, social work, human services, or a related field (or equivalent combination of education and experience).

Fluent in English and Spanish (oral and written).

Experience working in a behavioral health, social services, or case management setting.

Cultural sensitivity and ability to work with diverse populations.

Strong organizational and communication skills.

Ability to manage multiple priorities while maintaining accuracy and compassion.

Familiarity with HIPAA, mental health confidentiality, and ethical boundaries.

Proficiency with basic computer systems and EHR platforms (e.g., TherapyNotes, Excel, Google Workspace).

Preferred Qualifications:

Experience in community-based mental health or Medi-Cal systems of care.

Knowledge of trauma-informed and client-centered care models.

Familiarity with county behavioral health systems and referral processes.

Prior experience in care coordination or outreach roles.

Work Environment & Physical Demands:

Hybrid work may include in-person outreach, home visits, or clinic-based tasks.

Standard office conditions and use of technology (phone, computer, copier).

May involve some travel between locations or to client homes.

Occasional evening or weekend availability based on client needs.
