Social Services Director (Personal Services)



Wage DOE: $30-$35/hour

Colonial Vista Post Acute is seeking a dedicated and compassionate individual to join our team as a Social Services Director. In this role, you will be responsible for assisting individuals and families in navigating complex social systems to access vital services that enhance their quality of life. You will work closely with clients to assess their needs, develop individualized service plans, and provide support or referrals to various community resources. As part of a collaborative team, you will engage with healthcare providers, social service agencies, and community organizations, ensuring that clients receive comprehensive support tailored to their unique circumstances. Responsibilities

  • Assist clients in assessing their social, emotional, and economic needs to create tailored care plans.
  • Provide guidance and support to individuals and families accessing health and wellness resources.
  • Develop and implement individualized care plans for residents.
  • Collaborate with healthcare providers and community organizations to coordinate services and referrals.
  • Advocate for clients in various settings including healthcare, legal, and community services to ensure their rights and needs are met.
  • Monitor client progress and adjust care plans as necessary, providing ongoing support and motivation.
  • Maintain accurate and confidential records of client interactions, service plans, and outcomes.

Requirements

  • Bachelor's degree in Social Work, Psychology, Public Health, or a related field.
  • Previous experience in social services, community support, or healthcare settings.
  • Strong interpersonal and communication skills, with the ability to engage effectively with diverse populations.
  • Knowledge of community resources and social services available to individuals and families.
  • Ability to work collaboratively within multidisciplinary teams.
  • Strong organizational skills and the ability to manage multiple client cases efficiently.
  • Compassionate and patient demeanor, with a genuine desire to help others improve their lives.
