Physical Therapist (PT) - Sonoma Post Acute

Location: Sonoma, CA
Pay: up to $70 per hour
Schedule: Full Time, Part Time, or Per Diem

Responsibilities

• Provide physical therapy services to residents in a skilled nursing setting
• Create and follow individualized treatment plans
• Work with nurses, physicians, and the rehab team to improve outcomes
• Document progress and ensure compliance with regulations
• Educate residents and families on therapy goals and techniques

Qualifications

• Current California Physical Therapist license
• Strong communication and teamwork skills
• Experience in skilled nursing preferred, but new graduates are welcome to apply
