Position Summary:

The Community Coordinator ensures that a wide range of outreach activities/collaboration with health and human service agencies and community agencies are conducted. Develops and maintains community liaison; builds and maintains coordinated referral network and collaborative, working relationships between community agencies and organizations and East Boston WIC.

Key Responsibilities:

Outreach & Networking:

  • Complete a minimum of 60 activities per quarter.
  • Continuously make efforts to contact new groups and organizations, and provide existing organizations with updated eligibility information.
  • Visit and follow up with contacts regularly.
  • Develop and maintain community liaison relationships with health and human services providers and advocacy organizations within the local service area.
  • Build an active, coordinated referral network and foster a collaborative working relationship between community agencies, groups, and the WIC program.
  • Provide WIC program education and outreach materials to local agencies and advocacy groups.
  • Represent WIC at community health fairs, workshops, and other public forums.

Administrative Tasks:

  • Submit weekly schedule of planned activities.
  • Update resource bulletin board with current information.
  • Collaborate with care coordinators at NeighborHealth and agency partners.
  • Maintain the Massachusetts WIC Community Network Database with community contacts made and outreach activities conducted within the service area.
  • Enter activities in the database monthly and document details in the ‘Comment Section'.

Quality & Customer Service:

  • Ensure a well-rounded variety of outreach activities, including provider visits, presentations, informal sessions to highlight WIC, health fairs, events, material drop-offs, and meeting attendance.
  • Interact with patients and other staff in a professional and respectful manner, reflecting the needs and concerns of individuals.
  • Consistently provide excellent customer service, making patients feel welcome and valued.

Qualifications:

Education:

  • Minimum: High School Diploma.
  • Associate's Degree preferred.

Experience:

  • 2 years of experience in community work or health and human services is desirable.
  • Knowledge of the community and neighborhoods served by EBNHC is a plus.

Skills/Abilities:

  • Excellent customer service and interpersonal skills.
  • Bilingual/bicultural candidates are strongly preferred.
  • Ability to work independently and reliably in a fast-paced environment is strongly preferred.
  • Exceptional organizational skills.
  • Sensitivity to the needs of the participant population.
  • A well-organized, systematic approach to work.
  • Excellent communication skills.
  • Availability to work a flexible schedule, including weekends and evenings, is essential.

Compensation:

  • $21.51/hr to $32.26/hr, based on experience.
